Registration Procedures

  All paperwork and the enrollment fee must be submitted

one week prior to the child's start date.

New enrollments are accepted any time of year if space is available.

     1.  Submit your non-refundable Enrollment Fee of $65 to reserve your child's spot

     2.  Submit your child's enrollment data using our online interactive enrollment form                  

             If the online form is not compatible with your computer, you may print out a hard copy of the form

             and fill it out by hand.  If you do not have access to a printer, call us and we will mail you a form,

             or you can stop by to pick one up.  All enrollment forms will need to be signed by a parent or legal

             guardian in person one week prior to your child's start date.

     3. Two medical forms will be required to attend school:

          (1) Exam not less than two years old (Gold Form)

          (2) Current Florida Immunization Record (Blue Form)

          Please contact your child's doctor or the Health Department to obtain your forms

     4.  Children enrolling in our VPK Program* for the fall must also submit a VPK Certificate of    

          Eligibility (preferably prior to March 31st)
